The personal information requested on this form is collected under the authority of section 33(c) of Alberta’s Freedom of Information and Protection of Privacy Act (FOIP Act) and will be protected in accordance with Part 2 of the FOIP Act. The City of Lethbridge will use this information to respond to your information request. Non-identifying information will be used for statistical analysis and reporting purposes.
