Access to Information Request Form

This form is used to submit a request for information under the Access to Information Act (ATIA) to the City of Lethbridge. Many records are available without making a request under the ATIA. To determine whether you need to make a request under the ATIA or need help completing the form, contact the City of Lethbridge's ATI Coordinator for assistance.

The personal information collected through the Access to Information Request Form is authorized by section 4(c) of the Protection of Privacy Act to process your request for records or information.
